July 24, 2008 — The troubled saga of Mindy McCready added a new chapter in the last 10 days. The singer was admitted to a Nashville emergency room last week and has since signed up for rehabilitation, according to multiple media reports.

She appeared in the ER at Vanderbilt Medical Center on July 16 for undisclosed treatment.

Mindy made several emergency hospital visits in 2005. She was treated in May 2005 when her boyfriend allegedley beat and choked at her Nashville home. The following month, she was found unconscious in a Tampa hotel lobby after taking an overdose of drugs and leaving a suicide note. In September that year, she went to Music City’s St. Thomas Hospital after swallowing 30 anti-depressants on the heels of a phone feud with the same boyfriend.

Since last week’s ER stint, Mindy "has entered into an extended-care facility where she will receive the care needed," a spokesperson told People magazine.

Mindy’s troubles began in 2004 when she was arrested for buying drugs with a fake prescription at a Williamson County pharmacy. Problems continued to pile up with a drunk-driving arrest, multiple probation violations, an assault charge, a jail stay and the disclosure in April of a relationship with married baseball star Roger Clemens. In the middle of all the turmoil, she gave birth to a son, Zander, in March 2006.

Mindy was last arrested June 23 for misrepresenting the number of community-service hours she had served.

She agreed earlier this year to have her story told in a documentary. Its working title: Fallen Angel.
